"Add Comment" window is placed in ordinal cascade location when toolbar button is clicked and also relative to the mouse pointer when opened from context menu?
Position should better be in the middle of parent application window I think.

Position of windows is controlled by the window manager, not by the application. For example on Windows it's always the cascade location. On Linux you can change the default position of windows in system settings.
Centering would only make sense if Add Comment was a modal window. Since you can open many such windows simultaneously, they shouldn't be displayed in the same position.

I know it is one of those things, the default behavior... however I still think that if on my 24" LCD or even if I have Webissues on my secondary screen, that Add Comments window is way off popping up on primary screen top left corner...
Hope you understand what I mean...
Ok, that's a different thing. I'll try to fix it so new windows appear on the same screen as the active one.
